If my application is approved and a contracting relationship is established, I understand that I will operate as an independent contractor, subject to the terms of any applicable written agreements. I further acknowledge that Advantage Plus does not guarantee compensation, leads, sales opportunities, carrier appointments, or any specific level of income or business production.

I understand that no independent contractor relationship is established solely by submitting this application.

I acknowledge that I am responsible for maintaining all insurance licenses, qualifications, certifications, and continuing education required to conduct insurance business in the states where I operate.

I understand that submitting this application or receiving preliminary approval from Advantage Plus Insurance Agency Inc. does not authorize me to solicit, negotiate, sell, or transact insurance products unless I have satisfied all applicable licensing, carrier appointment, product training, and regulatory requirements.

I agree to comply with applicable federal and state insurance laws, carrier rules, advertising requirements, and suitability or best-interest obligations, including those applicable to life insurance and annuity recommendations.

I understand that carrier appointments and contracting approvals are subject to the applicable insurer's review and discretion.

I acknowledge that I have been provided access to the Advantage Plus Insurance Agency Inc. Privacy Notice and understand that the personal and professional information submitted through this application may be collected, used, stored, and disclosed for legitimate contracting and business purposes.

These purposes may include identity and licensing verification, application review, carrier contracting, compliance administration, communication, and other activities described in the Privacy Notice.

I understand that my information may be shared with authorized insurance carriers, contracting organizations, licensing verification providers, and service providers as necessary for these purposes, subject to applicable privacy and data protection laws.

I acknowledge that electronic signatures and electronic records may have the same legal effect as handwritten signatures and paper records, where permitted by applicable law.

I confirm that I have access to an electronic device, internet connection, and email account capable of receiving, reviewing, downloading, and retaining electronic documents.

I understand that I may request paper copies or withdraw my consent to electronic records and signatures by contacting Advantage Plus Insurance Agency Inc., subject to applicable law and any consequences for continued electronic processing that are disclosed to me.
